Size/dimensions - finished size required - see tolerances
Thickness -  (1/4", 3/4", etc.)
Color - (peach, clear, black...) see samples
Pattern - (seeded, German antique, ...) see samples
Edgework - (beveled, pencil edge,...) - see edgework
Application/Use - (tabletop, shelves, ...) - with the wide array of glass & glass related products available - we want to be sure you will be using the most practical/safest product for your purpose. 
Customer information - Minimum of name, zip code (so we may include accurate shipping/delivery charges), & contact information (phone, email or fax)
Note: We cannot give accurate quotes by phone or email on most custom installed residential & commercial products. We provide free, no-obligation estimates in your home or business, within our service area.

Description of Vehicle - (year, make, model, 2-door or 4-door, sedan, hatchback, convertible, etc.)
Glass needed - (driver's side door glass, windshield,...)

Installed price or glass only - We install within an approximately 50 mi. radius of Reading, PA. Convenient mobile service is available.
Insurance information - Required if placing order.  Just give us your policy #, company & agent name, amount of deductible,  your name, address & contact info. & we'll handle the claim for you.
